Each step is illustratedat:

I work with Adobe Photoshop (PS) so you have to adjust my indications to match your graphic software commands.
So, this being said, let's get started:
STEP 1
Create a blank document with the following characteristics:
Width: 283 px Height: 340 px Resolution: 72 dpi
STEP 2
Add a new vertical guideline at 40 px (You can do this easily in PS by pressing ALT+V and then )
This line will be the border between the front cover and the spine
STEP 3
This step is the most creative and time- consuming step. You'll have to actually do the layout of your cover.
When you're finished, your image should look like a plain 2 dimensional cover with the spine aligned to the front cover.
When you're happy with what you have designed, hit ALT+F. This will flatten the image so the layers will be no longer available.
STEP 4
Now you'll have to select the actual front cover. Click on the rectangular selection tool and click and drag from the lower-left corner of the image up to the upper margin and to the guide you've set before.
Copy the selection to clipboard (CTRL+C)
STEP 5
Save the following template to your hard disk and open it in PS. Get it from: http://www.affiliatecovers.com/10steps/4.jpg
You can get more templates like this for FREE from http://www.absolutecovers.com/link.pl?1=magic2001
STEP 6
Paste the clipboard content into the new document. Pressing CTRL+V should create a new layer
STEP 7
With the new layer active click on Edit -> Transform -> Distort (ALT+E A D)
This should let you to move each corner of the layer independently.
Drag each corner on the corresponding corner of the template. When you're done, hit ENTER.
STEP 8
Repeat steps 4, 6 and 7 with the spine side of the image.
STEP 9
While the spine layer is selected, click on Image -> Adjustments -> Brightness and Contrast (ALT+I A C) and set the brightness to -50 so it will look like the spine is shadowed.
STEP 10
Add a new layer, on top of all layers, set it's opacity to 50% select the 5px round blurred brush, select the white foreground color and draw a line over the edge between the front cover and the spine so it will look like there is a reflection of light on it.
Now, just save the image in .jpg or .gif format and you're done.
